Forms
NOTE: This is a sample form as a part of our Website Style Guide. If submitted, this form will not be received by anyone.
Forms are useful when data needs to be gathered for a specific purpose, whether it is instead of a PDF version of a form, or whether it is a survey. The form title will be shown above where it says "Form" in H3.
When you are linking to a form, have that form open in a new browser window. This ensures that users can easily navigate back to where they came from, even if the form you're taking them to lives in a section other than the one you're working in.
